Accessing your medical record

You can ask to see the information we hold about you. This is called a Subject Access Request (SAR).

Under UK law, you have the right to:

  • check if we hold your information
  • ask for a copy of it

The easiest way to see your health information is through the NHS App: https://www.nhs.uk/nhs-app/setting-up/.

What Shrewsbury PCN can provide

Shrewsbury PCN only holds information linked to services we provide. Your full GP record is held by your GP practice.

You can ask us for:

  • a copy of your records
  • records from a specific time period
  • information about a condition or treatment
  • electronic or printed copies

How to make a request

Complete a Subject Access Request form (download in DOCX or PDF format) and send it to us:

  • Or hand it in at Shrewsbury Health Hub reception at Severn Fields Medical Practice, Sundorne Road, SY1 4RQ

You can ask reception for a paper copy of the form if needed.

Proof of identity

We may ask for proof of identity before sharing information with you. This helps keep your information safe.

Requesting records for someone else

You can make a request for someone else if you have the right to do so. This includes:

  • with their permission
  • for a child
  • if you have legal authority
  • for someone who has died

You may need to provide supporting documents.

Children and young people

If you are under 16, you may be able to make your own request if you understand what it means.

Young people aged 16 or 17 can usually make their own request.

If a child cannot make the decision, a parent or guardian can apply.

How long it takes

We will respond within one month of receiving your request. If your request is complex, this may take up to three months in total.

Will I be charged?

You will not usually need to pay. We may charge a fee if:

  • the request is clearly excessive, or
  • you ask for extra copies

Can any information be withheld?

In some cases, we may not be able to share all information. This includes if:

  • it could cause serious harm to you or someone else
  • it includes confidential information about another person

Before you send your form

Please check you have:

  • signed and dated the form
  • included ID if needed
  • added any supporting documents

Incomplete forms may be returned.

Freedom of information

Information held by the PCN may be available to the public under the Freedom of Information Act 2000. This can include details about how the PCN operates, its services, and its governance arrangements.
