Support through your GP practice

Social Prescribing

Book a face-to-face or phone appointment via your GP practice. Social prescribers offer three-month support connecting you with local community resources.

They can help with:

  • Low-level mental health concerns
  • Isolation and loneliness
  • Connecting you to community support and peer groups
  • Access to Calmer CafΓ©s and Grief CafΓ©s

Mental Health Practitioners (Ages 18+)

Book appointments through your GP practice for short-term support with:

  • Anxiety and mood management
  • Sleep hygiene and coping skills
  • Initial assessments and onward referrals

MPFT Community Psychology Team

Support for common mental health problems including anxiety and depression. Self-refer for:

  • Chronic Pain & Illness Support: Help when chronic conditions impact your mental health
  • Improving Sleep: Evidence-based CBT techniques, mindfulness and relaxation for better sleep
  • Referrals available via your GP practice for: Managing emotions and understanding trauma
